Important Fee Information

MAKE CHECKS PAYABLE TO:  UASP

If your work affiliation is paying your registration fee, please make sure you indicate that payment will be coming from them.

Members: To qualify for the member rate you must either be a current (2009-2010) member of UASP at the time you send in your registration OR you must send in your membership fee with this registration (UASP- $50 regular/associate, $15 student, $25 retired). You can send one check for both. 

Student: To qualify for student rates you MUST include an advisor signature on the registration form. Student status will be confirmed with College/University rosters.

Group Registrations: Agencies and organizations submitting registrations for ten or more attendees may pay the member pricing if all the registrations are submitted together and the payment is received by 10/02/2009

Individuals wishing to attend only one day of the conference will pay the same fee.

THERE WILL BE NO REFUNDS AFTER October 2, 2009
Cancellations before October 2, 09 will receive full refunds.
